Forms
Most of what you need to do you can do via My AMP; like switch your investment options, nominate a non-binding beneficiary and update your profile, including contact details, mail preferences like going paperless. There’re only a few things you can't and you'll find the forms for those things below.
- Allocated pension application form – to set up an allocated pension for your retirement
- Additional insurance cover (Form 905985) - Use this form to apply for additional Death and, if applicable, TPD insurance cover on your SignatureSuper® account.
- Beneficiary nomination (Form 80000.1) – use this form to nominate a beneficiary, or change or cancel an existing beneficiary nomination
